feat: blame view

Scope
New /api/blame/:path route (git blame --porcelain) plus a blame panel showing per-line commit/author/date, with click-through to that commit.

Acceptance criteria

• Opening blame on a file shows correct attribution per line.
• Clicking a line jumps to that commit in the tree/diff view.
