First user-reported bug (#2).

The README + QUICKSTART both promise *"pauses for you to solve captcha
and click Submit"* — but the autofill function had `await context.close()`
in a `finally` block that fired UNCONDITIONALLY, closing the browser
right after autofill regardless of whether the user had a chance to
review.

User report:
  > Chrome opens, I see the screen being filled, but then it
  > automatically closes. If Chrome is not supposed to stay open
  > kindly let me know how to review the applications.

They were right. The promise wasn't being kept.

Fix:
- agent/backend/services/autofill.py: when status is one of
  {ready_for_review, captcha_required, verification_required}, print a
  clear "Browser staying open" banner to stderr and block on `input()`
  until the user hits Enter. THEN close the browser. KeyboardInterrupt
  + EOFError are caught so Ctrl-C aborts cleanly.
- Added `os` to the module-level imports so `os.environ` works at
  runtime (it was only used inside one function and Python doesn't
  validate global lookups at compile time, so this would've crashed
  only on the wait path).
- INSTAPLY_AUTO_CLOSE=1 escape hatch for unattended scheduled runs
  that should never block on stdin.
- README + QUICKSTART updated to describe the new "browser stays
  open, hit Enter when done" flow + the auto-close env var.

Verified: ruff clean, py_compile clean, apply_now.py imports clean.
