Re-land of #110, which was merged then reverted: it was built against the pre-APA-44 outbox.New signature and broke go vet on main. This version rebases onto the bounded-publish dispatcher and passes the full suite.
What changed vs #110: infraNewDispatcher now passes the APA-44 publishTimeout. The value mirrors cmd/api's production 30s rather than a shrunken test-only one, so the Pub/Sub-down case pays a real publish deadline instead of proving a fiction.
Test-only. Four infra cases live-proven: duplicate convergence, Pub/Sub-down, corrupt GCS integrity, Mockoon-down. Reports the APA-44 dispatch-stall defect rather than hiding it.
