Simplify Todo workflow and refresh cards - #127

Force-merge the newest open PR (#127) on top of main. main only contained PR #126 (single-flight extract job + /todo-extract/status) since this branch's merge-base; per decision, #127's simpler generate + card-refresh design wins and #126's extract-job set is reverted. Keeps #127's tree verbatim (= local latest implementation) and records main as merged so #127 is mergeable. Signed-off-by: yujiezhang-ops <yujie.zhang@novita.ai>
