docs: reconcile governance and current project state

Purpose

Reconcile governance/current-state documentation with verified merged repository and production state.

Scope

• Preserve DEC-2026-09-30-001 as historical record; add DEC-2026-09-30-002.
• Mark governance rollout implemented and verified in COO records.
• Synchronize PROJECT_STATE to current main HEAD and Article fix: use icon.png as the sole favicon source #62.
• Append reconciliation to CHANGELOG_AI.
• Remove legacy role wording that creates authority ambiguity with Human Owner → CEO/CIO → COO → Specialized Agents.
• Runtime code unchanged.

Verification basis

• PR docs: establish CEO/COO AI repository governance #93 governance CI run #704: PASS; Vercel verification PASS before merge.
• PR content: add eBay + local AI agents architecture guide #95 CI run #729: PASS; Vercel preview READY before merge.
• Current production Vercel deployment for main HEAD 54b363f...: READY.
