This program is written with HTML, PDFtk, Composer, CSS, and PHP. The purpose of this project is to create a new patient paperwork website for clients of Little Tesla where clients can fill out the new patient paperwork on a webpage and that data is flattened and then put into a pdf and sent to our office manager in a nice PDF Format.
