The 277.5010.X212.xml map (277 Health Care Claim Acknowledgement, 277CA)
ships in pyx12/map/ and is already covered by test_map_if_load, but it was
never registered in maps.xml, so the loader/validator could not resolve it
("Map not found. vriic=005010X212").
Add the index entry. fic=RA matches the map's GS01 valid code (RA), and
ST01=277 / vriic=005010X212 identify the transaction and implementation.
