fix(v0.7.34): keep document and resource identities consistent

Summary

• Release Context 0.7.34 with whole-document identity fallback and fixed resource identities.
• Restart pagination on body authorization fallback; never mix body pages or switch identities for embedded resources.
• Try the official same-identity image preview after explicit download permission denial, mark preview provenance, and retain unavailable-resource warnings.
• Improve knowledge query instruction reuse, symbolic version checks, and citations at the inspected revision.

Validation

• Workspace typecheck and lint passed (existing lint warnings only).
• Build and release preparation passed.
• 51 targeted identity, resource, report and evidence tests passed.
• Completed all default-suite chunks; corrected the single outdated preview-denial fixture and reran all 7 tests in that file successfully.
• 39 full-only integration tests, Node distribution smoke tests, and 2 CLI artifact tests passed.
• Dry-run package audit passed for all 13 public release packages.
• Reviewed both commits and full diff for public distribution; fixtures use synthetic documents and resource identifiers.
