grok-4.6 is a per-family catalog entry, not a widening of the unknown-model subset. grok-4.5 and composer stay on low/medium/high.

Self-review of this branch.
The grok-4.6 catalog is a dedicated low/medium/high/xhigh set via exact model id. grok-4.5, grok-composer, and the unknown-model subset stay on low/medium/high. Validation rejects xhigh on 4.5.
No blocking issues in this diff.
Merge this before #493. Both touch the effort catalog.
