Correct 0.2.1 agent truth and approval evidence - #48

Current Service output derives the Rails favicon and PWA icon pair while leaving only the selected-iOS AppIcon stock. Teach that boundary without freezing a Project-bound GapSet digest, and bind the resulting package plus published CLI 0.2.2 identities before the required two-turn smokes.
Teach the current Web-icon versus iOS AppIcon boundary and stop synthetic evaluations from treating project-bound GapSet digests as reusable release constants. Bind the resulting deterministic package bytes for the unpublished candidate.
Bind the exact unpublished package, CLI, Service, Plan, two continuing sessions, and their mode-specific terminal outcomes. Keep publication, promotion, deployment, and real GitHub effects outside this observation.

The Publication smoke used strict fake GitHub transport and made no real GitHub change. The direct-output smoke created one local 198-file application with no Git repository and zero Publications. This PR does not publish the plugin, move npm dist-tags, promote the public catalog, deploy the Service, mark the candidate ready, or merge it.
