Record why GOV-APPROVAL autonomy failed for todo2code (variables without scan-direct matrix), ship the operator guide and ordered refactor plan, and scaffold tickets 050-052 for unowned root paths, CI wiring of github-event-log, and AGENTS.md checklist promotion. All remain PLAN / WAIT_FOR_APPROVAL.
Cross-link twin-probes publication.gate (diagnostic probe, not trust root), validator freeze dispatcher, and skills-agent process. Record external PR landings and GitHub Actions outage as a publication blocker.
